29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On or about October 17, 2018, at the above addressed jobsite, employees performing heating ventilation and air conditioning operations from the roof were exposed to falls, of fourteen (14) feet four-tenths (4:10) inches, created by the presence of unprotected sides.

On or about October 17, 2018, at the above addressed jobsite, employees conducting heating ventilation air conditioning (HVAC) operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

On or about October 17, 2018, at the above addressed jobsite, employees were exposed to slip, trip and fall hazards when the bottom of the ladder was not kept clear of debris, including but not limited to, pallets and building materials.
