OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to ergonomic hazards.     a) Production Line 1 - On or about 10/17/2018, the employer exposed employees to musculoskeletal disorders (MSDs) and ergonomic hazards, in that employees were throwing empty boxes on an overhead conveyor with 7.5 feet height of conveyor belt above the floor for performing the same tasks repetitively.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) Production Lines - On or about 10/17/2018, employees were exposed to struck-by, crushed by and electrical hazards, in that no energy control procedures were developed and implemented for control of hazardous energy to maintenance employees when they were repairing the machines such as, but not limited to, Roach conveyors.   Repeat -  Menasha Packaging Company, LLC was previously cited for a serious violation of the same occupational safety and health standard and hazard of 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection # 979130, Citation #1, Item #1 and was affirmed as final order on 8/26/2014, with respect to a workplace located at 1645 Bergstrom Rd. Neenah, WI, 54956.
