An employee was fatally injured when the all-terrain fork truck overturned.      a)  The TEREX SS-842C all-terrain forklift was not operated on level or stable ground while employees engaged in re-roofing activities were lifted in/working from the work platform attachment on the forklift.   b)  The employee operating the TEREX SS-842C all-terrain forklift was allowed to walk away from the forklift while employees engaged in re-roofing activities were lifted in/working from the work platform elevated approximately 27 feet.
