An employee was working in an energized panel and was not trained on the appropriate use of Personnel Protective Equipment or Deenergization  of the panel. On or about 10/15/2018.

29 CFR 1926.97(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table E-4 and Table E-5.    Location:     485 Lexington Avenue, New York, NY 9th Floor.     Rubber Insulating gloves were not subjected to periodic electrical testing.  Rubber electrical gloves with a tested fate of October 2006 were not recently tested as per the Tables.  On or about 10/15/2018.
