29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     a) Jobsite, 215 W. Loyalhanna Street, Ligonier, PA 15658: On or before October 22, 2018, employees were working unprotected and exposed to fall hazards of approximately 27 feet while installing roofing components on a 5/12 sloped roof.

Loyalhanna Street, Ligonier, PA 15658: On or before October 22, 2018, an employee operated a JLG 6036 Forklift to lift roofing materials onto a roof without first being trained in its use, limitations, and inspection requirements.
