29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a. At the laboratory on or about 10/08/2018, a channel lock tool that was not approved by the gas manufacturer, was used to attempt to open the knob on a liquefied butane tank, as stated in section 3.4.9.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): a. At the establishment, employees working on tasks such as, but not limited to, calibration tests were exposed to chemical and/or fire hazards. Employees were provided and required to wear safety glasses and fire-resistant lab coats, on or about October 8, 2018.
