29 CFR 1910.147(d)(4): A lockout device was not attached to all power sources:    a) Production, 138 West 21st Street, Hazleton, PA - When cleaning dough from either of the two (2) mixers, employees placed each mixer's disconnect knife switch into the open position and did not attach a lockout device. The employees were exposed to caught-in and crushed-by hazards inn the event of the possible unexpected restart of the equipment, on or about October 24, 2018.    b) Production, 138 West 21st Street, Hazleton, PA - When cleaning dough from the various stations of two (2) Rondo production lines, employees did not attach a lockout device to the production line's energy isolating device or attach a lockout device to the male end of the power cord or female end of the electrical power supply cord, which were not constantly kept under the employee's immediate and exclusive control. The employees were exposed to caught-in and crushed-by hazards in the event of the possible unexpected restart of the equipment, on or about October 24, 2018.    In accordance with 29 CFR 1903.19, abatement certification is required for the violation.
