29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) Entire work site, ground area, on or about 10-24-18:  Two employees were below other employees who were working overhead, without head protection.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) North side of building, on or about 10-24-18:  Three employees were working on a scaffold while climbing over the ladder jack brackets for access and egress, exposing the employee to a fall hazard of 11 feet 2 inches.
