29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about October 25, 2018, during commercial construction activities including but not limited to installing soffit and fascia panels on a gas station canopy, the employer did not ensure employees were protected from possible head injuries by ensuring head protection was worn during exposure.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about October 25, 2018, during commercial construction activities including but not limited to installing soffit and fascia panels on a gas station canopy, employees were exposed to fall hazards of approximately 16.5 feet. Employees were not protected by an adequate guardrail system, safety net system, or personal fall arrest.
