29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and not constructed and loaded in accordance with that design:     Location: Exterior 2nd story above  single family residential home, two car garage; North West elevation     On or about: 10/4/2018     a) The employer was not qualified and constructed and used a job-made wooden platform, without guardrails or fall arrest systems, mounted on a GradeAll rough terrain forklift as a personnel work platform.  Employees were installing a second story window on a two story single family home, working from the platform at a height of approximately 14 feet in height with no fall protection, thereby exposing the employees to a fall hazard that resulted in severe injury/death.

On or about: 10/4/2018  Location: Exterior 2nd story above two car garage; North West elevation   a) The employer used a job made wooden platform, without fall protection, as a fork mounted personnel work platform not professionally designed and engineered for rough terrain forklift use.  Employees were working from a job-made wooden platform elevated by a GradeAll 534D-9 rough terrain forklift approximately 14 feet in height with no fall protection, thereby exposing the employees to a fall hazard that resulted in a fall injury.
