29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On the east side of the construction site, employees were not wearing hardhats while working below other employees that were installing framing material and were exposed to struck-by hazards from falling objects.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On the east side of the construction site, employees engaged in framing activities used pneumatic nail guns and were not wearing eye and face protection exposing them to struck-by hazards and eye injuries.

29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position.   On the east side of the construction site, employees cut construction material using skill saws with the lower guards wedged in the up position exposing the employees to amputation hazards.

On the east side of the construction site, employees engaged in framing activities operated Makita skill saws powered by an extension cord that was missing the ground pin, exposing the employees to electrical shock hazards.

On the east side of the construction site, employees engaged in framing activities worked without fall protection above 6 feet and were exposed to unprotected fall hazards of approximately 11 to 12 feet.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.     On the east side of the construction site, the employer did not provide an extension ladder where employees could access and egress the building upper landings exposing them to a potential fall hazard.
