29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.  Jobsite: On or about October 26, 2018 the end of each scaffold platform greater than 10 feet in length extended over its support more than 18 inches exposing employees to fall hazards of 10 feet, 1 inch.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Jobsite: On or about October 26, 2018 the employer does not protect its employees from fall hazards inherent in performing residential framing work by verifying compliance that fall protection training had been completed for employees exposed to fall hazards while performing framing work by preparing a certification record documenting that the training was conducted.
