29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.   a) On or about October 26, 2018 at the address of 50 NE Catawba Road in Port Clinton, OH:  The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) On October 26, 2018, at the address 50 NE Catawba Road in Port Clinton, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system;  a)  On October 26, 2018, at the address of 50 NE Catawba Road in Port Clinton, OH: The employer did not ensure employees were protected from fall hazards while replacing roofing material on a low slope commercial type roof structure.  Employees operating in this condition were exposed to approximately a 9 foot fall to the ground below.
