29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:    On October 26, 2018, the employer did not ensure that employees working in aerial lifts were restrained within the aerial lift buckets to prevent ejection, exposing employees to an unprotected fall hazard at approximately 20 feet.
