Charter Plastics, Inc. - On or about 10/25/18, and at times prior thereto, employees were conducting servicing and maintenance on equipment that included, but was not limited to, side winders, extruders, and re-grinders.  The company did not have specific lockout/tagout procedures for the equipment.

Charter Plastics, Inc. - On or about 10/25/18, and at times prior thereto, employees were performing servicing and maintenance on equipment that included, but was not limited to, side winders, extruders, and re-grinders but were not performing periodic inspections at least annually of the lockout/tagout procedures with all authorized employees.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:    A.)  Charter Plastics, Inc.; break room - On or about 10/25/17, a 240 volt electrical box (P119) had 3 boxes of plastic pipes and a stretcher box being stored in front of it.  The panel box powered the lights, water fountain, lab devices, and vending machines.
