29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   a. Commercial Roof: Employees engaged in roofing activities were exposed to a fall hazard of approximately 17 feet from an unprotected skylight without the utilization of a personal fall arrest system, covers or a guardrail system, on or about 10/26/2018.     b. Commercial Roof: Employees removing skylights were exposed to a fall hazard of approximately 17 feet from an unprotected holes without the utilization of a personal fall arrest system, covers or a guardrail system, on or about 10/28/2018.  c. Commercial Roof: Employees engaged in roofing activities were exposed to a fall hazard of approximately 17 feet from an unprotected holes without the utilization of a personal fall arrest system, covers or a guardrail system.  On 10/30/2018, an employee fell through an unprotected sky light and suffered fatal injuries as a result of the fall.
