Commercial Roof: Employees engaged in roofing activities were exposed to a fall hazard of approximately 17 feet from an unprotected skylight without the utilization of a personal fall arrest system, covers or a guardrail system, on or about 10/26/2018. b. Commercial Roof: Employees removing skylights were exposed to a fall hazard of approximately 17 feet from an unprotected holes without the utilization of a personal fall arrest system, covers or a guardrail system, on or about 10/28/2018. c. Commercial Roof: Employees engaged in roofing activities were exposed to a fall hazard of approximately 17 feet from an unprotected holes without the utilization of a personal fall arrest system, covers or a guardrail system. On 10/30/2018, an employee fell through an unprotected sky light and suffered fatal injuries as a result of the fall.
