29 CFR 1910.303(b)(1): Examination. Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees.     On or about 10/30/2018, Long John Silver's / A&W did not examine the electric equipment and ensure it was free from recognized hazards that are likely to cause death or serious physical harm to employees. The Russel Cooling Unit inside of the International Cold Storage Co. Cooler had the following hazards:  a)  The junction box on back of cooler not fastened securely.  The box appeared to be attached by one screw.   b)  Two hot wires going into the junction box were not protected by conduit and a metal clamp connector or similar device. c)  Two ground bonding jumpers on front of cooler that attached the covers were loose.

29 CFR 1910.303(b)(7): Mechanical execution of work. Electric equipment was not installed in a neat and workmanlike manner.  On or about 10/30/2018, Long John Silver's / A&W did ensure equipment was installed in a neat and workmanlike manner. The Russel Cooling Unit inside of the International Cold Storage Co. Cooler had the following hazards:  a)  The junction box on back of cooler not fastened securely. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about 10/30/2018, Long John Silver's / A&W did ensure a listed or labeled relocatable power tap was used or installed in accordance with instructions included in the listing or labeling.  Inside of the office/break area, a relocatable power tap was hanging above a doorway.  The relocatable power tap was used as a substitute for fixed wiring and provided electrical power to two cable boxes, a stereo receiver and an Ice-O-Matic Ice Maker.
