29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    JOB SITE: 945 N. Berry Road, Kirkwood, MO 63122, on or about 10/30/2018, the subcontractor workers were actively installing a new roof of the residential home.  The owners of Cascade Construction or the controlling contractor did not conduct an inspection of the job site, material or equipment.  The following conditions existed at the time of this inspection:    1)  A worker was not wearing eye or face protection while operating a pneumatic nail gun.  b2  Two of three workers on the roof were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.

The workers were exposed a struck-by hazard.

Berry Road, Kirkwood, MO 63122, on or about 10/30/2018, workers were observed on a 10:12 pitch, approximately 11' 3" above ground level, actively installing a new roof, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.
