29 CFR 1910.22(a)(3): (3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    a) On 10/26/18, the employer did not maintain a clean walking-working surface of an oil changing pit in that there were several oil leaks including from a drip tray used to capture the oil from the floor.  There is a hose with a rag attached to the tray that is leaking onto the floor below where the employees stand to change fluids from the vehicle causing the floor become slippery in the pit area.

29 CFR 1910.106(g)(1)(i)(f): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be stored or handled within a building having a basement or pit into which flammable vapors may travel, unless such area is provided with ventilation designed to prevent the accumulation of flammable vapors therein.    a) On 10/26/2018 the employer did not provide proper ventilation to prevent the accumulation of flammable vapors in a pit while utilizing 2 1/2 gallons of a Category 1 flammable liquid (Brake Cleaner).  Three employee were in the process of cleaning the floor of the pit area utilizing Brake Cleaner, a floor scrubber and squeegees during this process the vapors ignited causing a flash fire resulting in  burns to the three employees.

No delivery of any Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall be made into portable containers unless the container is constructed of metal, has a tight closure with screwed or spring cover, and is fitted with a spout or so designed so the contents can be poured without spilling.    a) On 10/26/2018 an employee transferred a Category 1 flammable liquid (Brake Cleaner) from a 55 gal drum into a 2 1/2 gallon plastic container that was not bonded nor grounded during transfer.  The 2 1/2 gallon plastic container was utilized for chemicals other than the brake cleaner.

29 CFR 1910.106(g)(7): Drainage and waste disposal. Provision shall be made in the area where Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), are dispensed to prevent spilled liquids from flowing into the interior of service station buildings. Such provision may be by grading driveways, raising door sills, or other equally effective means. Crankcase drainings and flammable liquids shall not be dumped into sewers but shall be stored in tanks or drums outside of any building until removed from the premises    a) On 10/26/18, three employees were cleaning the floor of the pit area utilizing a Category 1 flammable liquid (Brake Cleaner) utilizing a floor scrubber and squeegees.  Employees using the squeegees where pushing the mixture into a sump located in a corner in the pit, during this process the vapors ignited causing a flash fire resulting in burns to the three employees.

Employees were interchanging chemicals in the same bottles and containers.
