29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing roofing operations.  This was most recently observed on October 31, 2018 at 2601 Tulip Lane, Bellevue, Nebraska when 5 employees were working from a roof height of over 10' and they were not using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer is failing to protect employees from fall hazards while performing residential roofing activities without being trained.  This was most recently observed at 2601 Tulip Ln, Bellevue, Nebraska on October 31, 2018 when 5 employees were installing roofing materials on a residential structure and they had not been trained to identify the hazards associated with working from an elevated surface.
