29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.      (a) Worksite      On or about 11/1/2018, the employer did not inspect an excavation/trench over four feet in depth to eliminate hazards such as sections of concrete and spoil piles being within inches of the excavation edge before allowing an employee to enter the excavation for the purpose of removing a water line pipe. Further, the employer did not confirm that the water line from the street was no longer under pressure, which resulted in the trench filling up with water once the old section of pipe was removed by the employee in the trench.
