29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a)Weathergard Roofing and Crane Service, Inc. at 1519 4th Street Northeast, Watertown, SD 57201: On or about and at times prior to October 30, 2018, the employer did not ensure that each employee was protected from falls from heights in that, four employees were conducting roofing operations on a flat roof with no fall protective systems in place.  This condition exposed the employees to a fall hazard of approximately 15 feet 10 inches.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a)Weathergard Roofing and Crane Service, Inc. at 1519 4th Street Northeast, Watertown, SD 57201: On or about and at times prior to October 30, 2018, the employer did not ensure that each employee engaged in roofing operations on a commercial building, had been trained to recognize the hazards associated with working at elevations over  6 feet. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevation of approximately 15 feet 10 inches.

This condition exposed the employees to a fall hazard of approximately 15 feet 10 inches.
