29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it:  a) At the site - employees were working at 17 feet high from a carpenter bracket scaffold installing fascia, when the scaffold collapsed, creating a fall hazard, on or about October 31, 2018.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a). At the site - employees were working 17 feet high from a  carpenter bracket scaffold installing fascia, were not trained to recognize hazards by a qualified person, on or about October 31, 2018.
