29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about October 30, 2018, at a job site located at 701 Vista Ave SE, Massillon, Ohio 44646; during structural framing of a new addition to a commercial location, the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 20 feet by properly using a fall protection device.
