Where employees provide their own protective equipment, the employer shall be responsible to assure its adequacy, including proper maintenance, and sanitation of such equipment.  On or about 10/18/2018 at the 18 Kane Street Remodeling Project,  an employee was using his own personal fall arrest system that did not provide protection from contacting the ground while working from an aerial lift.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about 10/18/2018 at the 18 Kane Street Remodeling Project,  an employee was working from an aerial lift using a personal fall arrest system that allowed contact with the ground below.
