29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about November 6, 2018: Employee was observed collecting trash from the ground while other employees were working on the roof of the building performing roofing activities using tools and moving materials, and the ground employee was not provided with a hard hat to protect him from falling debris or in case a tool or material falls from the roof. Employee was exposed to a struck by hazard.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On or about November 6, 2018: An employee was observed operating a pneumatic nail gun while installing roof paper underlayment membrane and was not provided with a pair of safety glasses. The employee was exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about November 6, 2018: Four employees were observed performing roofing activities such as bit not limited to installing roof paper underlayment membrane and replacing plywood on a roof with a pitch of 5:12 were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee s provided with an alternative fall protection. Employees were exposed to a 16-feet fall hazard.
