29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems or personal fall arrest systems:    JOB SITE: Tropicana Lanes, 7960 Clayton Road, Saint Louis, MO 63117, on or about 11/6/2018, workers were observed  on top of the entrance overhang, approximately 10' 10" above ground level, actively painting the metal trim, and were not protected from falling by guardrail systems, safety net systems or personal fall arrest systems.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally:    JOB SITE: Tropicana Lanes, 7960 Clayton Road, Saint Louis, MO 63117, on or about 11/6/2018, workers were observed on top of the entrance overhang, working from an aluminum extension ladder, approximately 10' 10" above ground level, and tops of a non-self-supporting ladder were not placed with the two rails supported equally.  The workers were exposed to a fall hazard.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders as necessary, which would enable each employee to recognize hazards related to ladders and train each employee in the procedures to be followed to minimize these hazards:  JOB SITE: Tropicana Lanes, 17960 Clayton Road, Saint Louis, MO 63117, on or about 11/6/2018, the employer failed to adequately train each employee on the use of ladders as necessary, which would enable each employee to recognize hazards related to ladders and train each employee in the procedures to be followed to minimize these hazard.  The workers were not able to recognize hazards and the procedures to reduce or eliminate hazards.
