29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)Bag Machine (H-D7), MD Folder: On or about November 7, 2018, and at times prior, the employer exposed employees to caught-in hazards while removing excess bags from under the folder machine, in that employees crawled below running machines exposed to moving belts and/or conveyors to remove excess bags which could not be removed with the rake-like tool provided.
