29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:   On or about November 8, 2018, during the replacement of a storm water retention pipe at 4480 West 220th Street, Fairview Park, Ohio, the employer did not ensure an employee working inside a trench, with a varying depth of approximately 10 to 12 feet and with 70 degree or greater vertical walls, was protected from cave-ins by an adequately designed protection system.  The soil was classified as; Cohesive Soil, type B.
