29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     On or about November 5, 2018, at a job site located at 8431 Strausser Ave, Massillon, Ohio 44646; during the installation of roofing material at a new construction single family home, the employer did not ensure all employees using an aerial lift were protected from fall hazards approximately 10 feet by a fall protection device.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about November 5, 2018, during the installation of roofing material at a job site located at 8231 Strausser Ave, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a new construction single family home were protected from a fall of approximately 11.5 feet by utilizing a fall protection system.
