On or about November 1, 2018 in the hammer mill, an employee was exposed to a fall hazard while standing on an unguarded platform to grease the drag chain.     b. On or about November 15, 2018 on the pellet mill walkway, an employee was exposed to a fall hazard when walking on the walkway that had missing midrails to access the different levels.    c. On or about November 15, 2018 on the roof catwalk, an employee was exposed to a fall hazard when walking on the catwalk that had missing top rails to clean out the bird tank top drag.    d. On or about November 15, 2018 on the bird mixing area roof, an employee was exposed to a fall hazard when walking on the channel iron mounted to the roof to check the tank levels which had no fall protection system.
