29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) 135 S. Center St., Orange NJ    An employee opened the #2 elevator door on the second floor, without the elevator car present, exposing employees to a fall hazard of approximately 20 feet without fall protection.  On 10/23/18, a housekeeping employee fell approximately 20 feet into the elevator shaft after the doors were open without the elevator car present.      Condition occurred on or about 10/23/18
