29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)  On or about December 4, 2018 throughout the facility, employees were exposed to struck-by hazards while operating a Toyota forklift SN: 68267, Komatsu forklift SN: 307410, Nissan forklift SN: WGF03A45V that had parking brakes which did not work when fully engaged.
