29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   At Sand Hills Pipeline MLV 11 Phase 3: On or about November 10, 2018, and at times prior thereto, workers were exposed to a fire hazard, while operating a T1660 Mechanical Trencher that cut into a natural gas pipeline crossing which had not been identified/located prior to the excavation.
