29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about November 9, 2018, at the Utility Lines, Old North Park, Phase III (Nottingham & Mistywood) Denton, Texas new sewer line installation project, the employer did not ensure a stairway, ladder, ramp or other safe means of egress was located within 25 feet where employees were working to egress when required. Employees were exposed to engulfment hazards.

On or about November 9, 2018, at the Utility Lines, Old North Park, Phase III (Nottingham & Mistywood) Denton, Texas new sewer line installation project, the employer did not ensure excavated soil was at least 2 feet from the trenches edge, exposing the employees working inside the trench to struck-by and/or engulfment hazards.
