29 CFR 1910.147(a)(2)(ii): Application of control. When servicing and/or maintenance which takes place during normal production operations and an employee is required to remove or bypass a guard or other safety device or an employee is to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle:    Employees were exposed to machine hazards associated with points of operations (cutting dies) and in-going nip points (rollers) when they engaged in servicing and maintenance activities on the:    a)  P-Series Press 722    b)  P-Series Press WP1001    c)  Die Cutter B30    d)  Die Cutter B10    The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to allowing employees to remove, replace and adjust die cutters on the aforementioned equipment. The machines were not shut down or turned off prior to engaging in this servicing and maintenance activity.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(2):  Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.    On or about November 15, 2018, guards on the Die Cutter B30 were not affixed to the machine. The guards located on top of the machine above the cutting and perforating dies could be swung open and would not stop the equipment. Expanded metal guards on the side of the machine's frame were held in place with red tape. Drive rollers and portions of the cutting and perforating dies were located behind the unaffixed guards.
