29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. a. On or before November 20, 2018, in the break room, the employer did not ensure that the Flag Multi-Purpose Fire Extinguisher (model: ABS-050-G) received an annual maintenance check/inspection. The dry chemical fire extinguisher maintenance check/inspection was overdue by 3 months. b. On or before November 20, 2018, at the end of the main hall, the employer did not ensure that the Flag Multi-Purpose Fire Extinguisher (model: ABS-050-G) received an annual maintenance check/inspection. The dry chemical fire extinguisher maintenance check/inspection was overdue by 3 months.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: 29 CFR 1910.1200(e)(1)(i): A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas); a. On or before November 20, 2018, at the workplace, the employer did not develop a list of hazardous chemicals using a product identifier that was referenced on the appropriate safety data sheet for hazardous chemicals such as, but not limited to, the following: Butane Lighter, Oxygen, CaviCide, and Lidocaine Hydrochloride and Epinephrine Injection.
