29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.): (a) Mesa Completion Services, LLC at 2500 Country Road, Rifle, CO: On and before November 19, 2018, the employer did not ensure that employee training included methods of observation that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.). Employees gauged Production and Frac Tanks containing produced water. The gauging process required the venting of the contents in the headspace of the tanks at the thief hatch. Venting of the tank headspace released hydrocarbon gases and liquids into the work environment. The employer did not provide effective training to the employees regarding the detection of flammable gases. This condition exposed the employees to a fire hazard.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: (a) Mesa Completion Services, LLC at 2500 Country Road, Rifle, CO: On and before November 19, 2018, the employer did not identify and evaluate respiratory hazards in the workplace. Employees gauged Production and Frac Tanks containing produced water. The gauging process required the venting of the contents in the headspace of the tank at the thief hatch. 29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5): (a) Mesa Completion Services, LLC at 2500 Country Road, Rifle, CO: On and before November 19, 2018, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemicals contained within. Production and Frac Tanks, containing produced water, were mislabeled with type 704 National Fire Protection Association labels. The employer had not implemented the use of Globally Harmonized System labels at the work site. This condition potentially exposed employee to chemical hazards, including but not limited to produced water.
