29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Lot #311: On or about 11/20/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in sheathing installation. Employees operating these tools were exposed to struck-by hazards which included plastic collated fragments, wood debris and 3-inch fasteners.

Employees were not provided a fall protection system or trained to recognize a fall hazard existed.
