29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Lot #311: On or about 11/20/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in sheathing installation. Employees operating these tools were exposed to struck-by hazards which included plastic collated fragments, wood debris and 3-inch fasteners.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Lot #311: On or about 11/20/2018, and at times prior, the employer exposed employees to fall hazards of up to twenty-seven feet, three inches, in that the use of personal fall arrest systems were not enforced while the employees were laying sheathing on top of a third story roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Lot 311: On or about 11/20/2018, and at times prior, the employer did not provide fall protection training to employees working on a 5:12 pitched roof at heights up to twenty-seven feet, three inches. Employees were not provided a fall protection system or trained to recognize a fall hazard existed.
