29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:  a) Warehouse- On or about November 6, 2018, the employer did not provide or ensure there was a safe means of access for employees walking in the Warehouse where mechanical handling equipment was used, such as the electrical pallet jack and forklifts.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:     a) Warehouse- On or about November 6, 2018, the employer did not ensure that an evaluation of the operator's knowledge and skill to operate the power industrial vehicles safely was performed every three years for each type of vehicle operated, including the Yale sit down forklift, Model #ERC05ORGN48TE084, Serial #E108V11477W, Datsun sit down forklift, and Yale electrical pallet jack.   No abatement certification or documentation is required for this item.
