29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a) On or about November 27,2018, Jacks Roofing, LLC, as a controlling employer, did not ensure that employees of sub-contractor Alfredo Ortiz-Estrada were safe from health and safety hazards by conducting effective frequent and regular safety inspections of the jobsite by a competent person, ensuring hazards were mitigated such as but not limited to, fall hazards and ensuring anchors were installed while employees conducted re-roofing operations on the residential communities leasing office building and the residential mailbox building.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 27, 2018, the controlling employer Jacks Roofing, LLC did not ensure the employees of sub-contractor Alfredo Ortiz-Estrada were protected from fall hazards while engaged in re-roofing construction on top of the residential apartment complex leasing office and residential mailbox building. Measurements taken on the leasing office building were approximately 10 feet at the lowest point and approximately 12.2 feet at the next highest point were employees were working. Additionally measurements were taken on the residential mail box building at approximately 9.2 feet at the lowest point and 11.6 feet at the highest point were employees were working.
