29 CFR 1926.602(a)(9)(ii): Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signalled that it is safe to do so:  At QT Project:  On November 28, 2018 and times prior to, employees exposed to a struck-by hazard, while the operator utilized a Bobcat E42 Mini Excavator for compacting the soil that had the operator cab covered with cardboard which had an obstructed view to the sides and the rear.  The operator did not have a back up alarm or signal person while backing up.
