29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  For the employees laying concrete block on or about November 26, 2018 and times prior thereto, the employer did not assure the area employees were working on the scaffold at a height greater than 10 feet was fully planked.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity).  For the employees laying concrete block on or about November 26, 2018 and times prior thereto, the employer did not provide adequate fall protection at the end of the scaffold exposing employees to falls greater than 10 feet.
