29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about, November 28, 2018, an employee conducting snow removal work was not protected from falling through a curb opening in the roof deck. This exposed the employee to a fall hazard of approximately 16 feet.
