Broadway & Osage Street, Saint Louis, MO 63118, on or about 11/28/2018, Employees were not effectively trained implementing electrical Safety-Related Work Practices, such a determining flash hazard boundary and determining arc flash hazard associated with high current electrical service lines.  a) On 11/28/2018, employees were performing asbestos removal on the electrical conduits, were not trained to assess the arc flash level of the electrical service lines.

Broadway & Osage Street, Saint Louis, MO 63118, on or about 11/28/2018,  on or about 11/28/2018, employees working near 712v electrical conductors underground, did not use personal protective equipment such as, but not limited to Arc Flash rated apparel, face shield and voltage-rated gloves to protect against Arc Flash and electrical hazards.
