29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about November 6th, 2018, and at times prior, for the employees who were exposed to an overhead struck-by hazards from machinery, tools and materials while completing demolition work in the vicinity of 505 40th Street SE in West Fargo, North Dakota.

29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:  Or in the alternative   Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:  (a) On or about November 6th, 2018, and at times prior, for the employees who were exposed to a fall hazard of up to 12 feet while completing demolition work from a man basket that was attached to a telehandler that exceeded two times load center distance, Make JLG G-9-43a, Serial # 0160001091, in the vicinity of 505 40th Street SE in West Fargo, North Dakota.
