29 CFR 1926.102(a)(2): Eye and face protection equipment did not meet the requirements of ANSI Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:   On or about November 28, 2018, at 4605 E. St. Bernard Hwy., Meraux, LA, the employer allowed an employee to operate an Hitachi pneumatic nail gun while not wearing ANSI approved eye protection.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   On or about November 28, 2018, at 4605 E. St. Bernard Hwy., Meraux, LA, the employer failed to ensure two employees working from an aerial lift basket wore fall protection. The employees were installing fascia and soffit to the exterior of a home. The employees were approximately 20 feet above the ground.
