29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) New townhome construction located on Lot #44, Rear side: On or about 11/29/2018, and at times prior, The employer exposed an employee to collapse/fall hazards of up to 25-ft. to the lower level, in that the safety work platform used to frame the porch roof was not inserted into the sleeves provided or locked in to prevent movement during use.

(a) New townhome construction located on Lot #44, Rear side: On or about 11/29/2018, and at times prior, The employer exposed employees to fall hazards of approximately 25-feet to the lower level, in that a personal fall arrest system was not provided to an employee engaged in framing activities on a STAR Industries Safety Work Platform which was elevated by a JLG 8042 All-Terrain Forklift Truck.
