29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer:    a) On December 3, 2018, the Ultra Shore "S" Series Shield that an employee used for cave-in protection while working in a trench was not constructed in accordance with the manufacturer's recommendations and limitations outlined in the tabulated data, thereby exposing the employee to cave-in and struck-by hazards:    1) The shield system contained extensive damage on the northwest panel.    2) The shield system was constructed with three spreaders, missing the lower spreader on the south end of the shield system.
