29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces shall be kept in a clean, orderly, and sanitary condition:  a) Facility floors: The employer did not ensure that materials, metal scrap, metal beams, and metal framework were kept clear of the walkways in the facility, thus exposing employees to tripping hazards. 29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a) Facility: The employer did not ensure that the Clark S20 forklift was removed from service until restored to safe operating conditions. The forklift side to side function was in need of repair and was in operation.   This condition was noted on/about 28 JUN 2018.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section:  a) Facility: The Employer did not provide employees, voluntarily wearing filtering face piece respirator S-5687 Standard Dust Masks, with the information contained in Appendix D of this section ("Information for Employees Using Respirators When Not Required Under the Standard").
